Required Information

3. a description of where the allegedly infringing material is located on the Web site (product URL); and 4. information sufficient to permit us to communicate with you about the allegedly infringing material;
4. information reasonably sufficient to permit us to contact you, such as your address, telephone number, and e-mail address;
5. a statement by you that you have a good faith belief that the disputed use of the material is not authorized by the copyright or other proprietary rights owner, its agent, or the law;
6. identification of the intellectual property rights that you claim are infringed by the Site (e.g., “XYZ Copyright”, “ABC Trademark, Reg. 123456, registered 1/1/04”, etc.); and
7. a statement by you that the information and notices described above are accurate and, under penalty of perjury, a statement that you are the owner of the copyright or are authorized to act on behalf of the owner whose exclusive rights are allegedly infringed.
